Insurance Claim
GROUP PERSONAL ACCIDENT (GPA) INSURANCE FOR STUDENTS:
All students are covered under the GPA Insurance Plan which provides 24-hour worldwide accident coverage while they are at the school premises (including school-based Student Care Centre), in Singapore or elsewhere, participating in school-related activities, including Co-Curricular Activities (CCA) and sports. This contract is awarded to NTUC Income Insurance Co-operative Limited. It covers death, permanent and total/partial disability and medical due to an accident.

Claim Procedures
- For eSubmission, parents can go to Income’s GPA portal.
- Complete the online claim form
- Upload tax invoice(s), receipts (outpatient/inpatient claims)
- For inpatient claims– please submit the original tax invoice, receipts and Inpatient discharge summary/attending physician’s medical report to Income
- Confirmation email will be sent to parents

For manual submission, the Claim Form can be downloaded via NTUC Income’s website: https://www.income.com.sg/forms/claims/group-personal-accident-for-students-claim.aspx. Please note that this form requires the signature of Authorised staff of the School and School's stamp.
